Our innovative research and development team has validated the following improvement in our services for immediate release: Only when the first morning sample is collected thirty minutes after awakening will the diurnal cortisol peak be maximally seen. You should now instruct your patients to collect their first morning sample 30 minutes upon awakening for precise adrenal evaluation. When specimens are collected before or after the thirty minute mark, the cortisol curve is substantially altered (see graph below). As practitioners, we need the most accurate assessments in order to provide the best therapeutic results. For adrenal function testing, this new information will increase that accuracy.

You state that the morning sample should be taken 1/2 hour after awaking. Is that after being up and around or after waking? An example might be if the patient wakes and rests in bed for another 15 - 20 minutes before getting up and being active.
This is upon first awakening. Therefore, even if a person stays in bed lounging, but the brain is awake, the timer begins for the 30 minutes.
Testing AM cortisol levels at 30 minutes upon awakening will give you the highest peak in diurnal cortisol. As the Graph clearly shows, at 15 and 45 minute intervals upon awakening, you will still see a a spike in cortisol levels, however, it is not the maximal peak for each individual. When evaluating adrenal function, we are assessing the pattern of cortisol secretion as well as the highest (AM) and lowest points (PM), and these data points all contribute to our accurate assessment of adrenal function. For the optimal peak to be seen, saliva must be collected at 30 minutes upon awakening.
